First Visit Guide
Please arrive 10-15 minutes early for your appointment for registration and to fill in a health questionnaire.
When you come for your appointment, please bring the following:
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans, etc. and any other relevant information
- List of medications (if any)
If you need to cancel an appointment
- Call the office at least 24 hours in advance.
- We recognize that your time is valuable, and we apologize if we keep you waiting past your appointment time.

At CSOG, we accept Workers’ Compensation, Medicare, and most major insurance plans. Please contact your insurance company or you may call our office at to find out if we accept your insurance plan.
If your insurance plan requires you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, MasterCard, and Discover.
If we are a participating provider for your insurance company, we will bill them for any charges that are not your personal responsibility. See our financial policy for additional details.

To ensure transparency, this notice is to inform you that a credit card processing fee will be added to all credit card transactions, pursuant to Section 5-2-212, Colorado Revised Statutes, in the amount not to exceed the merchant fee that we incur processing your credit card payment.
Debit cards, FSA & HSA or prepaid cards do not apply.
Office visits: If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, the referral must be obtained before you can be seen for an office visit. Please coordinate this with your primary care physician in advance. Appointments will be rescheduled if required referrals are not received in the office by the time of your visit.
Physical therapy: You will need a referral from your physician or medical provider (primary care doctor, nurse practitioner, orthopedic surgeon, podiatrist, chiropractor, etc.) for insurance to pay for your physical therapy treatment.
CSOG will accept an initial referral by fax, by phone, or by email from your provider, and we will obtain the needed signed prescription within days of your referral, or you may bring the signed referral form with you to our office.
Prescriptions and refills for established patients of CSOG are routinely issued during our regular office hours (see Locations). You can make your request by calling our office at .
Please give our office at least 48 hours’ notice if you need a refill of your prescription during the work week. Call by noon on Friday for refills needed during the weekend.
No refills are issued during the evenings, weekends, or holidays, as the doctors do not have access to your medical records during those times. It is helpful to anticipate your refill needs to avoid an interruption in your medication therapy.
